Insurance Information

On the day of your first appointment, we ask that you provide all of your insurance information so we can file your claim for you. We will go over what your insurance is expected to cover, and you will be responsible for any overages. We ask that you familiarize yourself with your insurance coverage so you know what to expect financially. After 30 days of your appointment, you will be responsible for any balance left on your account, whether or not your claim has been processed. Please remember that we are not responsible for any actions taken by your insurance company, and we are not in contract with most insurance companies. We file insurance claims as a courtesy and therefore are not responsible for any errors. Additionally, we cannot guarantee coverage by your insurance company.
